It's a few years since I last travelled to the USA with my husband (both non-US citizens, different surnames). We've always approached the CBP agent together, and I'm fairly certain we've only completed one customs form. Is the "two surnames = two forms" something that was brought in recently?
And, as others have said - this is crazy (and shows a high degree of ignorance of other cultures). There are lots of countries where the concept of a "married name" simply doesn't exist and where people don't even have the
option of changing their surname to that of their spouse. Are US authorities really not aware of this?
