I think you're going to have to do the booking over the phone, and ask that they waive the reservations fee. I'd also request that they put some remarks in the PNR to cross reference your wife's PNR.
Would it make any difference if both tickets were on miles?
If I understand your response correctly, I can book a ticket for my son with my own miles.
