Can't check in online or at kiosk - can anyone help troubleshoot?

A few months ago, I stopped being able to check-in online or even at the airport via kiosk. The ticket counter agent would have to check me in (I believe to check ID). They would tell me nothing on my reservation was flagged and all of my name/birthdate information was correct so they could not tell me why I would be having a problem.

This started happening about the time that I think TSA made a change in the information they collect and also I started using American Express Travel instead of Carlson Wagonlit Travel to book. Also, I think one time a flight was booked with my middle initial and not full name - so the problem could have originated from any of those things.

I went through the DHS TRIP program to get a redress number, and my status shows closed (but I haven't received the letter yet telling me what the resolution was). I tried entering in my redress number to check-in but that does not work (may be a delay or who knows).

US Airways and American Express travel both have looked at my ticket, frequent flier information, and profile and can't find any differences - both have full name and birthdate. One person said maybe it's b/c my credit card has just my middle initial and maybe b/c Amex Travel puts a company phone number in the wrong field instead of mine but others say only first, middle, last name, gender, and birthdate matter. I've even had US Airways re-ticket my reservation and that STILL doesn't work. However, what's odd is the last few times at the kiosk when I check in by typing in my first and last name, it DOES let me check in (but by frequent flier number it doesn't and online still doesn't work).

Can anyone think of what I can do to be able to check-in online? Flying 2-3 times per week makes this a HUGE inconvenience and not being able to get on the upgrade list make me possibly have to switch carriers to see if I have a problem through them as well. I don't ever get ANY extra security and do not believe I am on the no-fly list - this seems to be some kind of issue with US Airways or between travel agent and the airline or maybe TSA got a different record with just my middle initial or something?
