I went to Costa Rica last year and I don't remember being forced to pay extra for insurance. I remember I was told I could either take some form of insurance (which I did since I was not familiar with their laws regarding auto insurance) or I could sign saying that I accept full responsibility and agree to immediately pay for any damages upon return.
