When it happened first time last fall I figured it was a glitch but twice in row is indeed suspicious. I wonder if LAX station need to run some sort of extra waitlist sweep in those circumstances.
Interstingly enough the upgrade list at the gate was relatively short (<20-25 people). I would expect it to be much longer on fully packed morning hub to hub 767 flight with all sorts of international connections ahead.
I actually asked 1K desk a few days prior to the flight if my name was on the waitlist and everything was fine plus the second IAD-xxx segment cleared just fine at 100 hrs or so.
I did OLCI the morning before, and it worked just fine. It was after another FTer mentioned the issues doing OLCI, that I went back to investigate. Got an exit row window (which is what I had on the 744)
