The best advice I've gotten re: CO and irrops is to try to be in a President's Club.

It can help a lot to have the dedicated desk agents who have fewer people to handle and may be able to take care of you more effectively than a phone agent or a harried gate agent who doesn't really care about elite cards, so much as clearing the 100-person queue in front of them and getting as many people home on time as possible.
