This is a popular question on this forum, and there are # of threads, such as
this one (found using term international license) which discuss the issue.
The bottom line: it is the LAW in Italy and thus yes, it is necessary. Will you ever have to show it, perhaps not. But in the event you are asked for it (at the rental car agency, if you are pulled over, in an accident) and do not have one then you may face serious consequences.
