I read somewhere that this practice started when there was a severe shortage of those paper loops that they used to put over the toilet seat after cleaning (they used to be in all the really fine hotels.)
Today the TP triangle is used by housekeeping almost universally to let you the consumer know that your bathroom has been cleaned & sterilized (even if you find things like "hair" when you first walk in)
