Midwest does. It's one of the things I really like about them. I've never bough the cellophane-wrapped BOB crap on the other US airlines but I'll spring for a meal on Midwest.

But the short answer is that it takes a galley big enough to reheat food to serve the meal in your picture. Many airlines would prefer to squeeze in a couple more seats.
