I can see how it happens with multiple tickets:
* BA requests a hold/auth code for the total amount of all tickets
* BA processes a transaction for ticket 1 at half the total amount
* BA processes a transaction for ticket 2 at half the total amount.

Since neither charge is equal to the amount of the hold, the hold + transaction never cancel each other out, meaning the charges stick and the held amount remains outstanding til it expires naturally.
