Similar experience to yours on JFK-PHX-ATL-LGA in early December. Site showed upgrade requested, and no-go at any of the gates.

edit: Point below is accurate. AX certificate is fine and is not UG-eligible, companion earns no miles. DL.com's handling of the certificate is deceiving
I used my certificate in Dec and we both got upgraded. Miss has no status. That's the second year in a row that me and the miss got the up on the cert. I have the reserve card that has the F option. Could that be the difference? Btw, both upgrades cleared at at the 24 hour mark which is in line with a requesting ups for non medallion.
