Long Answer: I did not requal for 1K for 2010 based on 2009 EQMs/EQSs. In Jan 2010 (while still technically a 1K), I flew over 10K BIS, completing the last trip on Jan 27 (i.e., AFTER the January sweep). When the CR1s did not show up after the Feb sweep, I called ... twice ... went to the supervisor on the second call. Consistently, I was told the policy was that if I had not requalified for 1K for 2010, I would not get the 2 CR1s for the January flying. HOWEVER, I was told that if I did requalify during the first quarter (by whatever means), I would get the CR1s during the sweep following my requalification.
I in fact had gotten a "requal offer" in early Jan 2010: Fly 5K BIS on UA metal in a "premium" fare bucket (e.g., F, A, C, D, Y, and some others), and you will requalify for 1K. I accepted the offer, flew those miles in late Feb, got my 1K back, and then "automatically" (i.e., no phone call or email required) got the 2 CR1s during the next sweep. My total BIS miles for that first quarter were under 20K, so don't know if I would have gotten a second pair of CR1s after requalifying, but I sincerely doubt that I would have.

I believe that even if you had flown more than 20K miles, you still could only get one pair of cr1s. I think that you only get one pair after you fly 10K miles in a quarter, but only get another if you fly 10K miles in another quarter. I flew nearly 50K ems in the last quarter of 2010 and only got one pair of cr1s.
