While I agree there are many reasons that it could happen, I can't recall once in 20 years/2M miles of travel that it ever actually happened. Perhaps I'm forgetting, but it seems to work like clockwork for me. I've had situations where no seat was assigned of course, as the aircraft was nearly full. But once I select/confirm a seat, I've been good to go.
