You always have to repeat yourself a bunch of times unfortunately, but be persistent.
TSA: Do you have any belt on, anything in your pockets? You're going through a scanner.
Victim: No, I'm not. I opt out.
TSA: Just go through the scanner there.
Victim: No, I opt out.
TSA: Do you wish to opt out?
Victim: I opt out.

TSA: Go on through the scanner. It's off.

I know the checkpoints I use for frequent flying and even recognize a good fraction of the TSA at each... they never seem to recognize / remember me though. Just be persistent on the opt-out request though and they will ultimately hopefully see the futility of invasive treatment of low risk passenger demographic.
