You monitor 121.5 in the cruise (if at all), not during approach.

I was an aircrew during my Navy years. We were well equipped radio wise and could monitor guard even with all radios on assigned frequencies. If guard had noise on it guard could be deselected and only the assigned channel would be available.

Example UHF or UHF+guard. As I recall it was the same on the VHF's. Not sure about the HF's.
