Sometimes it depends on the airline at that airport. I know at MHT, Delta, United and Continental exclusively use jetways, while US Airways occasionally will load from the ground.

Even there, it sometimes varies. At Atlanta, Delta is moving to all-jetways, but right now flights on C use jetways, while D is a mix of ground and jetways.
