Someone please correct me if I'm wrong, but doesn't the customs form also have an area for dates of when you left/came back?
The current standard US DHS customs forms do not have an area listing dates of when you departed from and/or returned to the country. The only date-related item that is filled in on such ordinary customs forms is the date the responsible party signed the form.
