I had the same problem. I hit the FTL reciently too, and had two longhaul flights (TXL to DTW and then to NRT) and my card only showed up after I arrived back from the last flight. I didn't have time to try to get lounge access but the features kicked in when my FRA to TXL flight was cancelled and I was able to call the hotline and rebook and get on an otherwise full plane.

Also even though I had FTL I would notice that my BP would still have M/M listed on it... which sort of annoyed me.
