I had a similar experience on an LDW rate. I had some parking lot damage to the driver's side door on a brand new Dodge Journey earlier this year at MCO. They just asked me to go to the booth when I returned at MCO. I didn't state and they never asked if it was for business or personal rental.

At the booth they just printed out my return with the damage noted and handed it to me. About 2 weeks later, I received a letter with a very simple form from National requesting more information. Date, location, other parties involved, brief description, etc. I did the damage myself, and explained (scraped a post while backing out of a parking space ). I returned the form as requested and that was the last I heard from them. They did not even request my personal insurance info. That was over 10 months ago.

The whole thing was quite painless. The worst part of it was that it happened early on in a week long rental and I had to drive the banged up car the rest of the week. It was embarrassing!
