First suspect would be weather - either turbulence, or winds aloft

Second suspect would be other traffic

Third possibility could be as OP suggests - some issue with the cabin pressurization system that's not sufficiently serious to ground the plane


Generaly, planes are more fuel efficient at higher altitudes The extra fuel used in the climb is outweighed by the overall savings
