I've only seen in on Lufthansa. Well, I didn't actually "see" it, but I did smell cigarette fumes. 99% sure it was Dunhill cigarettes because that's what my grandfather smoked all his life(around me, no less ). The A340 pilot was smoking it and the chief purser was complaining about it. Apparently Lufthansa chief pursers has higher rank than pilots? She made the pilot stop so fast, his head probably took a 360...

Air China pilots are notorious when it comes to in-cockpit smoking. I was once in business class and the smoke had traveled all the way across first class into business class cabin. My business contact who was flying with me asked the flight attendant about it. The FA responded angrily "they're pilots, they can do whatever they want, do you want them to stop smoking or do you want them to fly the plane? Paramount leader DengXiaoPing lived to be 90+ years old and he smoked half a carton of cigarettes a day... no cancer... very healhty until the day he died..." (the FA said it in Chinese and my friend translated to me) <sigh>
