I am flying my dog to the USA (BOS) from the UK (LHR) this coming Sunday with AA, on the same flight that I will be on.

I have an appointment with a vet on Saturday to get a Health Certificate and Temperature Acclimation Form.

Does anyone have specific details or an example of a valid Temperature Acclimation Form?

My vet is happy to fill in/sign the required documents, but hasn't previously had to issue a Temperature Acclimation Form (none of the vets within 30 miles of me have!).

All of the sample forms that I have found online are for use by USDA approved vets.

Any and all advice welcomed!
