I fly out of SAN often, and my 22" Tumi Alpha makes it through the bag police. However (when SAN-JFK was a 763) once onboard I had some difficulty fitting it in the AB/HJ bins. I've had no problems with the 752s, but as others have said, just make sure it doesn't "look" bulging or unnecessarily big or the bag police at the gate may notice.
And if you are forced to gate-check from SAN or JFK, the bags should be checked all the way to MXP. Have a great time in Italy!
