You say there were upgrades available but how many were available at any given time when you checked? Its quite possible that there were never 3 upgrades available at the same time for all 3 of you to be upgraded. There would have had to have been 3 upgrades available at the same time for your reservations to be upgraded.
I'd be willing to be bet there wasnt any system issue but just an issue in which the available inventory didnt match your needs.
