I went back and looked at mine again. The online version turns into the PDF version when you get ready to print it, and then the only optional response is for an e-mail address. Very confusing. Mine hadn't gone in the mail yet,so I went through it and filled in everything that was missing. I don't think they need to know where I work, heck that could change more than once in 10 years, but I don't want my application to be rejected. Getting a picture taken that didn't make me scream in horror was bad enough once!
