Actually it's not a huge deal, but when I've requested my shot of woodford and my seat partner banters on and on about how he wishes they'd bring back such and such cocktail of the month, I do get a bit annoyed. If they do a good job (or bad), I thank them -- that doesn't mean I'm going to waste 5 minutes of their time making small talk. Especially if some DM or PM might be sitting nearby getting angry about it
Regarding #8: I notice when people remove their shoes, they tend to try to make it less obvious to avoid ridicule. I really don't care as long as it's not visible (out of sight..). I only get upset when they begin to scratch their feet like they have athlete's foot or something. I know "class" doesn't have anything to do with seating, but seriously, keep the feet scratching out of my complimentary-upgraded personal space!
