Congrats on the CP! To answer your questions:

1. The CP card, technically, is required. But it hasn't been necessary as a practical matter. You'll book her flight and get a boarding pass at T-24...you're through.

2. No need to go through the name change thing until your second CP. She can keep her old driver's license with her maiden name for travel purposes. It's simpler that way.

3. The system won't throw up any erros. Still, what I would do is cancel all her prior reservations...then book her CP flights. Much easier than going through the cancel and re-booking one by one. Remember to track the canceled reservation numbers and TTF amounts as you cancel*.
