What airline are the IAH/FRA flights on? Is it possible that maybe the partner airline you were using did not confirm those flights and thats why they dropped off?
This definitely happens. Not all partners allow you to hold for the same amount of time.
Also, people make mistakes. Looks like no harm, no foul in this case. I wouldn't raise a fuss unless I was disadvantaged by the mistake, but that's me.
