in the interest of full disclosure, i did book the trip with two separate coupons MHT-DTW and DTW-STL. if anyone at DL can tell me they would have held the flight if i had booked it as a thru, i'll listen.
That itinerary clearly shows 1 stop & change of planes, so it's really not a "direct" flight but an issue of using the same flight #; there are a limited # of flight #s & this is a common occurrence. GA knew who you were because he knew he was missing one pax, and it's not the GAs call to hold the flight or not. In the interest of keeping things on schedule, DL often has to make the call to push, even if it means leaving an elite behind. Maybe there was another plane holding out for that gate, with other pax who had tight connections. Any # of reasons why they'll push & leave folks behind. Those times we've all made a tight connection we appreciate this!

Having a through-flight itinerary wouldn't matter, they would know they're missing you either way. Maybe somebody underestimated how fast you could run . Unfortunate that it turned out the way it did for you, however, on the last flight of the day.
