Yes you can use the Lounges
Yes you do have to pay the taxes on both tickets but only need to use one lot of miles

However, the 2for1 voucher can only be used for flights on BA aircraft, so i assume you are breaking the flights down and flying, MAN-LHR-LAX and then MIA-LHR-MAN ? if so you can use the lounges at MAN and LHR on the outbound and then MIA and LHR on the return including the arrivals lounge at LHR by going landside and then airside again before your connection.
