How things have changed. I recall an Air Europe flight on a 747 20 years ago to Bangkok from LGW when smoking was allowed. We were heavily delayed x2 - the first delay resulted in us being boarded but kept on the ground for a couple of hours whilst something was fixed.

Despite constant announcements stating smoking was not permitted, many pax were lighting up and trying to conceal the cigarrette, ie inverting it into the cup of their hand and frantically waving their hand about as they exhaled to try and disperse the visible smoke.

I can vividly recall even on the take-off run being able to smell fresh smoke and hearing a CC member yell out "sir, please put the cigarette out right now"
