I don't think you are missing anything. I find AC.com to be very hit and miss when it comes to pricing multi-city itin's as a through fare.

How are you entering the multi-city? If you are trying YYZ-YUL, YUL-YVR, YVR-SFO, you might want to try entering the cities differently such as YYZ-YUL and then YUL-SFO.

Also, do you need to book on AC.com? I too have had better luck with sites such as travelocity to book such trips (I don't usually book with Orbitz due to the pricing in USD, unless this has changed lately).
