When CO was still in SkyTeam, my card regularly failed to read. The agent at CLE kept it and mailed it in for a replacement for me. She printed me a form so I had it to access in the ~2 weeks it took for the replacement. They didn't charge me any fees. I wonder where it's written that they'll charge you for new card?
