When the liquid scare hit a few years ago, LHR got very strict with carry-on sizing for a while. Lines were longer, and even at Fast track, I was having mixed results bringing my Travelpro 22" bag on (whereas before, it was never a problem).
Then, a while back, things loosened-up, and since then, I have never had a problem bringing my 22" bag through this line. Since then, have not had anybody look at, or say one word to me about my bag. (And on average, I am through LHR T3 a couple of times a month). It still may be "random" but my experience in my last 20 or so trips through, has been no problem at all.
Also, in reference to other airlines at LHR, there have been many times I've gotten my bag through security, only to have a BA, IB, KLM, what-have-you, make me size my bag at the gate, and subsequently insist I check it.
I've been forced through the sizer in the Fast Track lane, though the last time was a while ago. I honestly think it just has to do with the whim of the person manning the line that day. There is a mix of elites and F/J passengers going through that line.
If you make it past security, I have never once seen anyone from AA checking baggage sizes at the gate so you should be in the clear. I fly out of LHR at least once a month on AA and have been doing so for several years, so I've seen plenty of examples.
