It is part of the disable passenger ferrying service. Usually it is for the disabled passenger and one travel companion. If they are not busy and there is space on the cart then more of the disabled passenger's travelling party may be accommodated.

Sometime they are used to shuttle late inbound passengers to catch connecting flights a gate far away.

My father usually tips them, it all depends on how helpful the services is and how friendly/helpful the driver is.
