Ironically, in W&B calculations, carry on luggage "weighs" less than if it ends up as Valet luggage. According to AC 120-27E, a carry on bag "weighs" 16 lbs and checked luggage weighs 30 lbs.
As far as the CPAP machine goes, another suggestion for you... get a doctor's note saying that it's necessary for you to travel with it in the cabin in the event you fall asleep. Claim ADA exemption.
Here is the FAA rule expressly allowing you to bring the CPAP machine onto the airplane for use:
