Really? It's been years since I've done a bus transfer between HND and NRT. I don't recall going via TCAT, but perhaps things have changed.
I'd suggest taking the Skyliner train from NRT to Nippori. From there, the OP can take the Yamanote line to, say, Hibiya. After looking around Hibiya/Ginza, get back on the Yamanote line to Hamamatsucho. Transfer to the monorail. Ride the monorail to HND.
