Just a brief update:
Somewhat predictably, I did not hear back from AMEX RE: credit limit increase within 3 working days.

I waited another couple of days and then cancelled my card in writing, requesting a confirmation of the cancellation. I also stated my reasons as to why I was to canceling, hoping it would trigger get some sort of response (possibly apology!). Negative.
Another 4 days later, I received mail from AMEX - an application for credit limit increase!!
As of today, 14 days after AMEX had received my cancellation request, still no written confirmation or call. I just called them now; card has been cancelled, and I can expect a letter within a week. Absolute shocker.
