They show you the next available slots, and then you can look forward if you would prefer the following week, etc.

I believe that the conditional approval letter is valid for 30 days, so you would need to complete your appointment by then, or else you would have to start the process over (and presumably pay another $100). But call to check for sure.
3) How long is the interview? If you only have 2 hours connection time at one of these airports, is that enough time? Do you have to exit security, have the interview and then re-enter security to connect to your flight at most airports?
2 hours should be more than enough time. Interviews are scheduled for 45 minutes but generally take less time. Mine took about 30 minutes but it was for NEXUS, so I had to be interviewed by CBSA and CBP. Others here have reported 20 minute interviews for GlobalEntry only.
Yes, you will have to exit security because the GlobalEntry enrollment offices are located in public areas of the terminal (so that non-traveling members can access them).

You can schedule your interview at ANY enrollment office in the country....it does not have to be the closest one to your home.
