Is there a handy-dandy COMBINED list of UA-CO-US clubs, including city, location at airport, and phone number? Preferably a pdf, but any format would be helpful.

At some point in the near future, of course, I imagine UA and CO clubs will be the same thing. I just hope that the merged entity will continue to honor my UA lifetime membership, purchased in 1979.
