It'll never happen ! ie by the time you get them to admit anything, the 3 or 7 day 'hold' on the card would have expired already.
or (b) the bmi software remembers it's already got approval for this transaction and just gets the revised ticketing information before adding the booking to the ticketing queue.
I think its more human error than the software!
