I'm not certain of this specific aircraft, but many newer ones have an indicator in the cockpit that shows the status of the doors, so if it had been armed for several minutes after arrival it may have been noticed by the pilots.
From the memo:
"Our investigation thus far reveals..<snip>..At some point, the forward service door was purposely re-armed and the slide intentionally deployed, endangering lives and equipment"
