This is clearly a software issue with the bmi booking system. It appears that they enter all the ticket details, process the credit card transaction, and then complete some internal processing (like adding the redemption to the ticketing queue).
In your case this final step seems to have failed for some reason. Probably some data missing from the ticket details. From your description there were several attempts to get whole process to complete (and hence several authorisations against your card).
There are two solutions to this - either (a) the bmi software
reverses the credit card hold when it fails to add the ticket to queue (I assume) and goes back to the start, or (b) the bmi software
remembers it's already got approval for this transaction and just gets the revised ticketing information before adding the booking to the ticketing queue.
Would be nice if people tested their credit card systems once in a while
