In ancient history before family names were invented, respect was shown by title before/ after the one & only name: SIR Lancelot, LADY Gwenivere(sp?), REV Tevye (Fiddler on the Roof,") ______SAN, DOCTOR Bob ( =Robert Smith MD, co-founder of Alcoholics Anonymous, where confidentiality forbids last names.)

Conversely, it is the greatest disrespect of all to call someone by Last Name WITHOUT any title: "Watson, come here, I need you!"

I suppose first names detract from the ambience of First Class, but I've been so seriously dissed elsewhere that I won't worry about it.
