1. Basically, you 'll have to call United to make the reservation, then take the vouchers to purchase the ticket at the airport or via ticket by mail. There are several links with info on VDB strategies and using the vouchers on the "FAQ, Wiki, and Glossary" sticky at the top of the UA forum with more detailed info.

2. You can't combine more than one book of vouchers, but can use all of the vouchers in a given book in one purchase. So once you gather multiple voucher books, just use them on different trips, book separate ticket for a companion, etc.
