When this happens, one needs to call the 1K desk and asked to get transferred to the 1K web support. Be prepared to sit on hold for a while as they take care of it. But it has worked so far for me.

And if you sponsor an upgrade, if it wasn't done from your account on-line, you'll need to make sure you authorize it correctly. I did this at the RCC in SFO for +1.
Same situation here except I specifically signed for the upgrade at the airport. Best advice as mentioned above is to get web support to fix as two calls on the outbound to 1k agent could not fix. Web support did it quickly. Upon the return (my boss no less on a trans-con) I got a web support agent who 'could not help' so I hung up. I called 1k back and requested a supervisor fix and was all set that way too.
