What I can answer:

You will have to have the ticket issued yourself; he can't do it at the airport in ATH.

It would cost $150 to cancel and redeposit, which you can do up until the flight departs.

You might be able to get a ticket on hold now and then only ticket it if you know that he needs it. I'm not sure of the point at which you're no longer able to hold an award ticket. Once you do ticket, it gets issued and is in the system very quickly, so I wouldn't expect a problem with that aspect of it on your friends end.

If I were in your situation, I would call US and explain the situation and ask for an agent or supervisor to guide you through how best to handle this.
