Had this last month...twice. Gifted a friend a pair of CR1s for a Hawaii trip and did all the paperwork at the airport to authorize both certs, got a panicked call the day before departure that he couldn't complete OLCI. Called 1K desk and agent had to get supervisor to re-authorize the upgrade. A week later got a panicked email from my friend when once more he couldn't complete OLCI. Another call to the 1K desk and another supervisor and the upgrade was re-authorized. On this call I was told authorization can now be done on line about which I had not known until I was told by the res agent.

I suppose catching this during the OLCI process is fortunate, but I can see this would be a real problem when wait listed on the day of flight.
