Exactly what happened to me

and what I had to do to (hopefully) get it fixed

so if you signed up for a Chase Continental debit card and get what you think is junk mail from Continental - suggest you review the contents thoroughly. If it is a new OP number you did not request, it is likely the same
snafu situation.
