Don't forget NRT-SHA. I flew that back in March of 1999. It's safe to say that it shifted to NRT-PVG upon PVG's opening and then is now gone.
SFO-SHA replaced NRT-SHA before PVG opened (around a year prior, iirc). NRT-PVG was never offered by UA.
There's a nice chart towards the bottom of the following wiki that puts most of the mystery to rest (assuming it is accurate).
